What You’ll Do:
As a key member of our care team, you’ll work under the supervision of a Case Manager to deliver direct personal care and support to patients at home. You’ll help patients with daily living activities, provide companionship, and monitor their condition—while keeping the care team informed every step of the way.
Your Day-to-Day Might Include:
• Assisting patients with bathing, dressing, grooming, and personal hygiene
• Helping with toileting needs and mobility support
• Performing light housekeeping duties, changing linens, and preparing meals
• Reporting food and fluid intake/output as assigned
• Taking and recording vital signs under professional supervision
• Providing medication reminders and emotional support
• Observing and reporting any changes in patient condition or behavior
• Documenting care accurately and communicating with the care team
• Supporting therapy exercises as directed
• Calling to confirm scheduled visits and respecting each patient’s routine

What You’ll Need:
• High school diploma or equivalent
• Current Rhode Island CNA certification (required)
• CPR certification (required)
• Valid driver’s license and automobile liability insurance
