General Summary/Responsibilities:
The Supervisor Ambulatory Care Management is a Registered Nurse, and responsible forthe supervision and provision of care through the Care Manager – RNs, for patient care management programs.In providing timely and appropriate clinical guidance, the Care Managersprovide a systematic organization of patient care with the Physician Practices, and coordinate with other care team members and/or settings. The Supervisor Ambulatory Care Management - RN will coordinate and provide high quality, patient centered care management to patients in need, as identified by high-risk registries. This will include, but is not limited to developing individualized care plans, care coordination across the continuum, prevention plans, and ongoing monitoring. The Supervisor Ambulatory Care Management - RN works with the Care team to provide efficient and effective coordination between the Primary Care offices and other specialty groups, and to educate clinical staff on the various program offerings.This position requires excellent communication and leadership skills, flexibility, strong clinical knowledge base, critical thinking skills, independent decision making and collaboration with the patient’s health care team. Additionally, requires strong computer skills, knowledge of Microsoft Office and the EPIC electronic health record system.
