The Speech Pathologist evaluates and then directly provides or supervises the provision of speech therapy services to patients in their homes or residences. When functioning in the capacity of the Home Care Case Manager (HCCM), the Speech Pathologist is responsible for the provision and coordination of all components of a patient’s home care services. The Speech Pathologist provides care as ordered by the physician, follow policies/procedures, and conditions of participation, and regularly reports to their supervisor. The incumbent facilitates the development and implementation of the plan of care as outlined by the physician. In addition, the incumbent is responsible for all required documentation related to the care they provide and communicates pertinent information regarding the patient to all appropriate health care team members.

The role of the Speech Pathologist/HCCM is to oversee the home care services provided to patients and, specifically, those services related to their field of practice. This position interfaces with all levels of home care staff as required by the Medicare Conditions of Participation and TJC
