What You’ll Do:
As a RN Home Health Clinician, you’ll manage a caseload of patients from admission through discharge, working as part of a dedicated interdisciplinary team. You’ll assess, plan, and coordinate care while monitoring progress and adjusting treatment as needed to promote the best possible outcomes.
Your Day-to-Day Might Include:
• Performing comprehensive assessments and developing personalized care plans
• Coordinating care and referrals across disciplines and specialties
• Monitoring patient progress and adjusting care to achieve optimal outcomes
• Communicating with patients, families, and providers to ensure continuity of care

What You’ll Need:
• RI Registered Nurse license (required)
• Bachelor’s Degree in Nursing or related field (strongly preferred)
