Family Nurse Practitioner (FNP) Highlights
This position is a contract assignment with potential to convert to permanent employment based upon attendance, performance, and business needs.
Work Hours: Monday – Friday, 08:30 AM – 05:30 PM.
Training will be conducted at the Palmyra Clinic Tuesday – Thursday, 08:30 AM – 07:30 PM.
Family Nurse Practitioner (FNP) Responsibilities
Provide primary and family evidenced-based care, including accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for patients.
Perform administrative duties including opening/closing the clinic, inventory, patient follow-up, verifying insurance, collecting payment, and maintaining a clean environment.
Manage patient appointments and perform routine examinations to ensure positive patient health outcomes.
Educate patients on healthy lifestyle choices, complete diagnostic tests, and coordinate care with other healthcare professionals.
Encourage patients to actively participate in their healthcare and wellness through services such as health screenings, behavioral health, and chronic condition management.
Collaborate with a dedicated clinical care team to drive better outcomes for patient panels.
Family Nurse Practitioner (FNP) Requirements
Master’s Degree level Family Nurse Practitioner program with current National Board Certification.
Active license to practice as an Advanced Practice Nurse in the state of employment.
Minimum of 1 year recent Primary Care experience as a Nurse Practitioner in either a family medicine or primary care practice preferred, or successful completion of CVS MinuteClinic primary care training program.
Working knowledge of quality measurement in chronic disease management.
Self-motivated, adaptable, and able to work independently.
Effective verbal, written, and electronic communication skills.
Outstanding organizational and multitasking skills.
Initiative, problem-solving ability, adaptability, and flexibility.
Proficiency with information management and technology.
Willingness to obtain multi-state licensures.
