Job Summary:
Responsible for providing Social Worker services to Integra’s patients and their families to address psychosocial, environmental and behavioral factors which impact the healthy functioning and/or recovery of the patients and families. Provides in-home counseling services when the patient is unable to access services outside the home. Functions as a member of the multidisciplinary health care team providing direct clinical social work services including assessment, counseling, discharge planning and coordination of services. Develops a network of community agencies to facilitate referrals. Tracks patients along with continuum of care, identifying patterns that have a negative cost-quality impact and assists with the development of plans for improving care
Duties and Responsibilities:
Provides competency, both individually and part of a multidisciplinary team, in performing clinical care and assessment of all patient populations.
Conducts initial intake with patients, and introduces patient and family to program services. Patiently answers questions about services in a friendly manner and in easily understood language, and assists patients to access them. Makes frequent patient contact, including meeting patients and maintaining telephone contact, and assists patients with support, referrals, and help in resolving any problems. Takes a pro-active vs. reactive role in planning of care and education, acting as liaison for patient and family in working with various agencies.
Conducts home visits according to program protocols. Visits home to properly assess the home environment, family support, parent/child interaction, and other pertinent family issues. Arranges for services needed, or refers to appropriate agencies. Follows through on referrals or services, and assists with any problems encountered
Maintains all required documentation, including full assessments in patient’s medical records, follow-up note, statistical reporting and any other required documentation by Clinical Supervisor or the Joint Commission.
Develops and maintains network of community agencies to provide support, patient access and services in an out-patient setting. Provides appropriate referral and advocacy when clinically called for.
Tracks patients along with continuum of care, identifying patterns that have a negative cost-quality impact and assists with the development of plans for improving care.
Enhances professional growth and development through continuing education.
Participates in development and implementation of programs, policies and procedures.
Protects and preserves patient and staff rights for confidentiality.
Performs all other related duties as assigned.
Requirements:
Master's degree in Social Work required.
Experience in health care or hospital setting strongly preferred.
Active LICSW required.
