Title: Physical Therapy Assistant
Location: Norwalk, CT
Salary: $60,000 to $70,000, DOE
Responsibilities:
– Works closely with licensed Physical Therapist in home care setting
– Documents notes in EMR system
– Performs routine physical therapy tasks
– Involved in performance improvement
– Adheres to all compliance and regulatory standards of organization
– Works closely with home health aides to deliver care
– Speaks with patient’s family about education and training
Requirements:
– Associate’s Degree in Physical Therapy
– Preferred experience in home care setting
– Experience with EMR systems
– Valid Connecticut driver’s license
– Must work well with staff members and patients
