PRIMARY OBJECTIVE
Under the direction of the Site Director, the Family Services Coordinator is responsible for enrolling children and families into the Child Development Lab Centers, as well as providing casework services, and counseling and referral resources. The Family Services Coordinator is responsible for working with the site director and other center staff to coordinate services relating to child and family wellbeing, including Disabilities, Health, Mental Health, Nutrition and Family Partnerships.
DUTIES & RESPONSIBILITIES:
·Enroll and recruit eligible children for the center, including children with disabilities. Ensure completion and submittal of paperwork in a timely fashion.
·Complete Family Assessments and make appropriate referrals to assist parents in the development and implementation of their identified goals. Arrange home visit schedules to meet family needs.
·Assist families with procuring required dental and medical records for children enrolled in the program; analyze medical records to determine special needs and requirements for each child. Follow-up with families to ensure medical and dental work required for children is updated and monitored.
·Review developmental screenings (example: Ages and Stages Questionnaire) and assist teachers in addressing parent and/or teacher concerns.
·Ensure all grant funding, licensing, and accreditation regulations, including Head Start Performance Standards, are met with regard to providing casework services, and counseling and referral resources.
·Identify social and emotional needs of families through direct contact, teacher reports, and child observations and provide support through counseling and referrals. Establish and maintain a relationship with contracted Mental Health Consultant to coordinate referrals, classroom
·and individual observations in a timely manner.
·Coordinate all screenings and services of children and families to determine appropriate course of action.
·Ensure children with special medical concerns have an action plan that has been implemented to address their needs.
·Respond to and work with external partners, including representatives from CYS (Chicago Children and Youth Services) and other funding agencies, licensing representatives, and the National Association of Education of Young Children (NAEYC) accreditors as needed. Respond to their requests for information, data, audits, etc.
·Coordinate scheduling and implementation of children’s individualized education plan (IEP) recommendations by meeting with teachers, families and Local Education Agency (LEA).
·Attend regular staff meetings and provide reports to staff and Site Director related to the health, mental health, and other special needs of children.
·Attend parent committee meetings and council advisory boards as it relates to specific topic areas and share information to staff and families at the centers.
·Create and update a Community Resource Directory and serve as the resource person for families to facilitate access to social services within their communities.
·Maintain and manage all children’s files and enters data into web-based tracking system.
·Provide regular tracking reports to District Office Staff, including the Special Assessment Services (SAS) Manager and Eligibility Specialist.
·Participate in the program self-assessment process and provide input into the creation of management improvement plans.
·Adheres to CCC Customer Service Excellence standards.
·Performs related duties as assigned.
QUALIFICATIONS:
·Bachelor’s Degree in Social Work, Psychology, Sociology or a related area of social science required.
·A minimum of two years previous work experience in providing services to families in Head Start programs, community mental health agencies or related early childhood programs. Experience working with children with disabilities preferred.
·Must be aware of pertinent legislative acts such as the Americans with Disabilities Act and Head Start Performance Standards and relevant resource material to provide the most appropriate services and referrals to families of children with special needs.
·Must be self-directed, innovative and able to assess current child and family needs in an effort to develop practical action plans and goals to improve family functioning.
·Excellent verbal and written skills required, as well as working knowledge of computer data entry operations.

The starting salary for this position is $69,545. Offered salary will be determined by the respective collective bargaining agreement and applicant's education, experience, knowledge, skills and abilities.
