Location:Camden, NJ
Position Status:Full Time (35 hours per week)
Salary:$21 to $22 per hour
Position Summary:
The Supportive Services Care Manager will provide case management services for residents of apartment sites and coordinate workshops and training sessions for residents.
Duties and Responsibilities:
- Organize and coordinate a network of formal and informal activities, services and supports to optimize the well-being of residents
- Provide direct case management and remote services to residents
- Perform specialized activities and functions specific to program. (See Program-Specific Responsibilities)
- Complete documentation and other reporting requirements
- Meet regularly with supervisor, and participate in agency collaboration team meetings
- Adhere to policies and procedures of CFS
- Provide and coordinate social services within the apartment complex
- Provide outreach and connect residents with the case management services provided on-site
- Coordinate referrals to community resources
- Coordinate the planning and execution of a minimum of on-site workshops/events that focus on an identified need of the resident community
- Develop with the Community Manager an identified space on-site for information sharing regarding community resources and services. Ensure the information table is kept current with brochures.
- Provide a monthly program report to the Apartment Manager.
- Conduct periodic surveys to further identify the needs of the population.
- Other duties as required.
Level of Service Expected:
- Provide a certain number of hours per week as specified by each site.
- Coordinate the planning and execution of on-site workshops/events that focus on an identified need of the resident community.
Outcomes Expected:
- 50% of residents participate in a case management activity within the year. (case management, workshop, etc.)
- 85% of clients assisted report achieving goal
Requirements:
- Certified Social Worker (CSW) or equivalent certification, if applicable
- Bachelor degree in a human services field, preferred. Experience may substitute for education on a year for year basis.
