The RN Care Manager – Inpatient would be working for a Major Hospital System and has career growth potential.
RN Care Manager – Inpatient Highlights:
•Schedule:M-F 8a – 4p with rotating weekends
•Pay Range:$55-60 Hourly
RN Care Manager – Inpatient Responsibilities:
The RN Inpatient Care Manager coordinates the care and services of selected member populations to facilitate a safe and timely transition to the next appropriate level of care. Clinically assesses, plans, implements, coordinates, monitors, and evaluates options and services to meet an individual’s health needs. Partners with the healthcare team to ensure all aspects of the patient’s needs—clinical, psychosocial, and financial—are adequately addressed to manage the patient’s timely progression of care and safe transition from acute care. Promotes effective utilization and monitoring of healthcare resources. Collaborates with the interdisciplinary team to build a comprehensive case management plan to achieve optimal clinical and resource utilization outcomes.
Essential Duties:
• Develops and maintains a comprehensive case management plan of care in collaboration with physicians and the interdisciplinary team.
• Identifies and refers high-risk patients to social services as appropriate.
• Implements proactive strategies to prevent readmissions and evaluates causes when readmissions occur.
• Develops and documents discharge/transition plans and ensures coordination and modification based on patient needs.
• Monitors and escalates financial management or payer-related issues impacting post-acute care planning.
• Facilitates patient progression toward discharge goals, participating in care rounds and addressing barriers to discharge.
• Coordinates transitions of care across the continuum, ensuring effective communication among patients, families, and care teams.
• Maintains knowledge of Federal, State, and regulatory standards, including The Joint Commission, CMS, and Medicare guidelines.
• Provides education and resources to patients, families, and care teams on utilization and care progression.
• Identifies and reports potential risk management or quality issues as appropriate.
RN Care Manager – Inpatient Qualifications:
Required Skills & Experience:
• 2 years of inpatient case management experience
• Active NJ RN license in good standing
• Excellent communication, negotiation, and conflict resolution skills.
• Strong data and computer proficiency.
• Knowledge of regulatory standards related to Case Management.
• Expertise in rapid cycle change or clinical performance improvement.
• Prior clinical experience in care and management of hospitalized patients.
Preferred Skills & Experience:
• Experience in acute care case management.
• Case management training from a professional Case Management organization.
• Case Manager Certification preferred.
• Previous travel experience preferred.
