Provides quality pharmaceutical care, and fills/dispenses medications in accordance with the latest pharmacy standard practices. Reviews prescriptions and medication orders for appropriateness and compatibility with current therapy, disease states, drug allergies, adverse drug effects, duplication of therapy and compliance to ensure patients' medication therapy is safe, effective, timely, and free from drug-related problems. Communicates with all healthcare professionals and patients concerning medical care to ensure safety and effectiveness. Provides direction to technical staff and participates in performance enhancement initiatives.Job Duties
- Prepares and dispenses medications for infant, pediatric, adolescent, adult, and geriatric patients.
- Evaluates evidence-based literature to offer appropriate clinical recommendations in order to optimize drug selection and individualization of therapy.
- Ensures and executes policies and procedures that ensure appropriate, cost-effective, and safe medication use.
- Performs clinical medication surveillance, documents interventions, reports adverse drug reactions and medication errors.
- Coordinates the activities of pharmacy technicians engaged in the preparation, labeling, and delivery of medications and other pharmaceutical supplies.
- Counsels/Provides drug information pertaining to medications, drug usage, and compatibility to physicians, nurses, other healthcare professionals, and patients in accordance with OBRA or other regulatory standards.
- Weighs, measures, and prepares sterile and non-sterile medication products utilizing appropriate tools and equipment.
- Acts as a resource or consultant to other healthcare professionals regarding appropriateness, availability, cost, and/or alternativeness to therapies.
- Doctorate Pharmacy or
- Bachelor’s Degree Pharmacy
- Ability to accurately dispense and monitor medications.
- Ability to work effectively in a team environment.
- Ability to make independent judgments under stressful situations at times.
- Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.
- RP - Registered Pharmacist - State of Pennsylvania Upon Hire
- 2 years relevant pharmacist experience or
- 1 year PGY1 pharmacy residency
- Familiarity with common pharmacy technologies, both software and hardware.
