We are seeking aClinical Program Coordinator (NP or PA)to support the growth and quality of our Clinical Services Team. This hybrid position combines50% leadership/supervisionwith50% direct patient care, allowing you to guide programs while managing complex post-acute patients.
The role includes25–50% travelacross Wisconsin and Midwest partner sites to ensure consistency, quality, and operational success.
Position Overview
The Clinical Program Coordinator oversees and enhances clinical programs, ensuring high-quality care and operational efficiency. Serving as the lead resource for assigned facilities, this role collaborates with providers, interdisciplinary teams, and leadership to drive clinical excellence and performance improvement.
Ideal candidates are experienced NPs or PAs who enjoy blending leadership with hands-on care, thrive in dynamic settings, and are committed to quality and collaboration.
Key Responsibilities
Clinical Oversight (50%)
Supervise, mentor, and evaluate clinical staff and students
Lead protocol implementation, performance improvement, and compliance initiatives
Oversee orientation and training for new providers and programs
Review documentation for accuracy and regulatory adherence
Support new program launches and business startups
Patient Care (50%)
Evaluate, treat, and manage patients in collaboration with physicians and facility staff
Coordinate care plans, monitor outcomes, and provide education
Facilitate discharge planning and ensure timely documentation
Develop standardized protocols and templates to maintain care quality
Leadership & Collaboration
Lead program development and improvement initiatives
Promote accountability, teamwork, and excellence
Represent the organization at meetings and industry events
Support strategic and budgetary planning
Qualifications
Licensed or eligible NP/PA in Wisconsin
5–8 years of clinical experience (hospital, post-acute, or complex care preferred)
Strong leadership and communication skills
Commitment to quality improvement and patient-centered care
Ability to travel 25–50%
Meets DOL “medium work” physical requirements
