POSITION SUMMARY:
As a Case Manager at our Residential Treatment Center, you will play a crucial role in the holistic development of youth by overseeing and facilitating life skill deliverables. In this dynamic and impactful position, you will be responsible for creating and implementing comprehensive case management plans tailored to each individual's needs, with a primary focus on equipping them with essential life skills.
Duties and Responsibilities:
Comply with funding source, contract, and or program's Annex A.
Complete required documentation within established timeframes per Federal, State, County, and agency regulations, policy and procedure.
Provide case management: facilitate group programming through evidence-based curriculums, provide individual and/or family supportive counseling, and provide referrals, advocacy, and linkages.
Participate in the development of service plans, comprehensive assessments, discharge planning, and case conferences.
Coordinate interventions and services provided by multiple agencies, including within CFFS programs.
Appropriately utilize supervision process and agency supports.
Participate in team meetings regarding the individual's behaviors and ongoing service needs.
Collects statistics and complete written reports as required.
May provide specialized activities and functions specific to program.
Life Skills Development:
Design and implement tailored life skills programs to enhance the practical and social capabilities of youth.
Provide individual and group coaching sessions, fostering independence and resilience.
School Enrollment and Academic Support:
Collaborate with educational institutions to ensure timely and appropriate enrollment for each youth.
Advocate for academic needs and support youth in navigating educational challenges.
Medical Support:
Coordinate and advocate for necessary medical assessments and treatments.
Collaborate with healthcare providers to address the medical needs of youth, ensuring a holistic approach to their well-being.
On-Call Support:
Participate in an on-call rotation to provide immediate support during emergent situations or crises.
Collaborate with the residential team to implement effective crisis intervention strategies.
Assessment and Case Planning:
Conduct comprehensive assessments to identify individual needs and strengths.
Develop and implement individualized case management plans, addressing life skills, educational goals, and medical requirements.
Collaboration and Communication:
Work closely with educators, healthcare professionals, and other stakeholders to ensure a coordinated approach to youth development.
Maintain open communication with families to keep them informed about their child's progress and engage them in the treatment process.
Documentation and Reporting:
Keep accurate and up-to-date records of all interactions, assessments, and interventions.
Generate reports and contribute to case reviews to inform decision-making and continuous improvement.
REQUIREMENTS:
Bachelor's level case manager with 3 or more years of relevant experience or an unlicensed master's level practitioner with 1-year of related experience.
