Manages patient’s entire journey through the organ transplant process, acting as the main point of contact for patientsw and their families

Bachelors degree in Nursing and a required Current licensure and registration (or valid permit) as a registered professional nurse with the New York State

transplantation in heart, lung, kidney, pancreas, and liver with some of the highest survival rates in the US. We are currently looking for a RN, Clinical Transplant Coordinator to join our growing rteam. As the RN, Clinical Transplant Coordinator, you will collaborate with physicians and other healthcare professionals, managing the clinical care of patients and families throughout the liver transplantation process

Position: RN, ClinicalTransplant Coordinator

Location: Bronx, NY

Hours: Mon-Fri – 8 AM-4:30 PM

Salary: $127,000-$158,000

Responsibilities:

Manages patient’s entire journey through the organ transplant process, acting as the main point of contact for patientsw and their families

Collaborates closely with transplant surgery, hepatology, and referring providers to optimize care coordination for patients with malignancies such as colorectal cancer and hepatocellular carcinoma (HCC).

Participates in tumor boards, coordination of multidisciplinary treatment pathways, and contribution to the development of protocols for managing complex medical conditions associated with liver failure.

Applies broad clinical expertise while serving as a clinician, educator, collaborator, and patient advocate.

Functions in a semi-autonomous, self-directed capacity, the coordinator provides both direct and indirect patient care in accordance with professional practice standards and evidence-based clinical guidelines

Requirements

Bachelors degree in Nursing and a required Current licensure and registration (or valid permit) as a registered professional nurse with the New York State

A minimum of two years of clinical experience in a highly specialized complex transplant care program. Spanish speaking preferred

Experience in transplant patient care role

Excellentcommunication skills, both written and oral
