Provides oversight and supervision to assigned colleagues and coordinates operations in the absence of, or as delegated by, the Social Work Manager, and/or Program Director/Administrator. Functions as a member of a multi-disciplinary team for a defined population. Assesses and evaluates social determinants of health, identifies needs and barriers, assists in care plan development and goal setting, and identifies and facilitates linkage to network and community resources for patients impacted by complex psychosocial situations.Job Duties
- Provides telephonic, video, and/or face to face outreach, assessment, evaluation, and intervention as indicated by evidence-based practice in response to complex referrals from multi-disciplinary care management colleagues and/or provider practices.
- Provides clinical supervision to assigned colleagues.
- Fosters a team approach by working collaboratively with all care management, practice, and community-based team members.
- Performs biopsychosocial assessments to identify and assists with barriers to receiving care.
- Assesses the need for community and/or governmental agency involvement and acts as a patient/family advocate.
- Identifies appropriate resources and coordinates referrals within the network and community to assist patients with complex psychosocial needs impacting their ability to meet their goals.
- Monitors and audits workflow and charting compliance as assigned.
- Organizes and leads discipline specific meetings within an assigned team.
- Collaborates with team members to assist in development of appropriate care plans addressing complex psychosocial and/or financial barriers to care.
- Builds relationships and establishes linkages with other departmental programs, other network departments, and community programs.
- Documents all outreaches and interventions succinctly and in a timely fashion in the medical record, in accordance with departmental and network policies and procedures.
- Promotes patient self-management and empowers patients/families to achieve maximum levels of wellness and independence.
- Master’s Degree Social Work
- 5 years in a healthcare and/or community setting with demonstrated supervisory, case management, and/or progressive administrative skills and/or responsibilities.
- LSW - Licensed Social Worker - State of Pennsylvania within 1 Year
- Ability to recognize the role of cultural, social, economic, and behavioral factors in the accessibility, availability, and delivery of public health services.
- Ability to adjust course of action and plans in relation to others' actions.
- Knowledge of community resources and services across multiple counties.
- Strong assessment, evaluation, and intervention skills.
