Summary
Provides hands on personal care and activities of daily living according to the aide care plan as directed by the Registered Nurse or Therapist. Provides other support services as a member of the care team.Job Duties
- Provides personal care and related activities within scope of practice for designated patients in strict accordance with the written plan of care.
- Assumes responsibility for and completes routine and complex assignments for assigned home care/hospice patients within the allotted period of time, including the completion of documentation of patient care provided and communication with other disciplines as appropriate.
- Observes changes in patient functional status and personal care needs; documents observations and reports changes and/or findings to supervising clinician or clinical manager as appropriate.
Minimum Qualifications
- High School Diploma/GED
- 1 year Experience in a healthcare setting as a nursing assistant or nursing aide.
- Builds strong relationships with patients, family members, caregivers, and members of the interdisciplinary care team.
- Knowledge of medical terminology.
- Strong time management skills.
- Ability to drive to patients homes, regardless of weather and traffic conditions, and manage schedule to meet patient needs.
- Ability to work in patient homes.
- Ability to work with and support terminally ill patients and their family members/caregivers.
