Assistant Clinical Service DirectorWe are currently looking for an Assistant Clinical Service Director to come on-board for our Home Health client in the Bronx, New York. The Assistant Clinical Service Director (ACSD), in conjunction with the Clinical/Center Service Director (CSD), supervises all day-to-day operations of the Interdisciplinary teams and participant outcomes.The ideal candidate will have a minimum of 3+ years of administrative experience in a management capacity in a certified home health agency, long-term home health care, acute care, medical-surgical, and/or critical care, nursing home experience, diagnostic & treatment clinic preferred.

JOB RESPONSIBILITIES:
- Oversees, coordinates, and provides support to site-based Managers and clinical staff, including but not limited to the DTC and Day Health Center, rehab department, nutritional services, social workers, and home services management.
- Accountable for coordinating Care Planning meetings as needed, ensuring all meetings are timely, monitoring any necessary follow-up after meetings, and serving as mediator when the team has disagreements regarding patient care treatments.
- Balances & allocates IDT and related resources throughout the site and ensures adequate team assignments and coverage.
- Facilitates, reviews, and ensures effective development and execution of personalized care plans that result in improved satisfaction & outcomes for all participants.
- Coordinates with the Centralized IDT and Care Planning Supervisors to ensure timely completion of assessments and care plans for each participant
- Provides oversight of the provision of services that impact participants’ care.
- Implement in-service training and management directives to support the development and advancement of team members.
- Collects and reviews data for operational metrics geared towards ensuring participants are treated in the right venue at the right time and at the right cost at the highest levels of quality.
- Participates in all quality assurance and regulatory compliance activities to ensure appropriate regulatory certifications.
Education:
- Baccalaureate degree in Business Administration, Healthcare Administration, or related fields.
- Degree from an NLN accredited school of nursing, BSN preferred.
- Successful completion of an accredited Nursing Program required.
- Completed NYSED approved infection control coursework. A minimum of 8.0 education contact hours that may include contact hours of mandatory training requirements such as infection control coursework.
License:Current active and unrestricted license and registration in New York State required as a Registered Nurse.Please note:This position is not open to 3rd party c2c agencies. Experience:
- Minimum of three (3) years of administrative experience in a management capacity in a certified home health agency (CHHA), long-term home health care (LHCSA), acute care, medical-surgical, and/or critical care, nursing home experience, diagnostic & treatment clinic preferred.
- Customer Service experience is required.
- Managed long-term care insurance experience beneficial.
- Minimum of one (1) year of experience working with a frail or elderly population or, if the individual has less than one (1) year of experience but meets all other requirements, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.
- Minimum experience of three (3) years of supervisory experience required.
License:Current active and unrestricted license and registration in New York State required as a Registered Nurse.
