Our client is one of the largest not for profit health systems in the North East and have been established for over 100 years providing care to patients across the five burroughs of New York and many other counties. We specialize in treating geriatric patients in various nursing homes and long-term care facilities. Our organization consists of over 2,000 employees and we do a great job providing all of our staff members with an opportunity to work in a collaboratively team-oriented atmosphere. As the RN Home Care Hospice Case Manager, you would be seeing patient’s in their home, hospital, or nursing facility. You would be providing the coordination of care for patients at the end of life. You would see roughly 5 patients per day. This would be a 35-hour work schedule and work hours from 9am to 5 pm Monday through Friday. Please see job highlights below.
Title: Home Care Registered Nurse Hospice Case Manager
Location: Bronx NY
Salary: $110,000 to $135,000 + full comprehensive benefits package
Responsibilities:
Assessing patient’s medical health and medication effectiveness
Documenting all notes necessary in EMR system
Reviewing orders from physicians about plan of care
Monitoring each patient and arranging follow up visits
Seeing patients in the field and providing on going assessment
Provide direct care to patients as prescribed in the Interdisciplinary Plan of Care in order to maintain the highest level of comfort and quality of life and assuming primary responsibility for case management.
Evaluate and perform ongoing assessment and revise initial written plan of care with Interdisciplinary collaboration weekly or as the needs and conditions of the patient/family change.
Authorize, coordinate and supervise care, as prescribed in the Interdisciplinary Plan of Care, with contracted vendors in order to meet the needs of the patient.
Supervise and maintain ongoing effective communication with other hospice personnel involved with patient care.
Knowledge of and availability to perform patient intakes and information visits as needed including explanation of the hospice benefit/Medicare, complete physical assessment, completion of all pertinent paperwork, and communication of new patient status to the HOV team.
Provide bereavement resources to the family as appropriate.
Assess home care needs, being aware of the physical, emotional, and spiritual aspects and gather data on social, economic and cultural factors which may influence health, well-being and quality of life.
Assist patients, family members or other clients with concern and empathy; respect confidentially and privacy and communicate in a courteous and respectful manner.
Requirements:
Associates Degree of Nursing (BSN Preferred)
At least 1 year of Hospice nursing experience
Driver’s license
New York state Registered Nurse license
Strong personal skills
Home care nursing experience a plus
Communication skills
