Program Summary: Better Future Wellness Center is a peer-run, peer-operated wellness center providing support, connection and recreation for adults with mental health issues who seek community, friends, social outlets, and relaxation. At Better Future Wellness Center there is a focus on health and wellness with specialized services and activities, including: support groups, social activities and shared meals.
Position Summary: The Care Manager is responsible for meeting with members to assess their needs and identify services needed. The Care Manager will provide referrals to services such as TANF, food banks, utility assistance programs, rental assistance and transportation assistance available in the member's community of residence. Members of the Better Future Wellness Center are mental health consumers.
Duties and Responsibilities:
Provide referrals and linkage to housing, financial, medical, educational, vocational, legal, spiritual and other needed/indicated community resources and services in support of consumer recovery and community integration to BFSCH members.
Provide face-to-face care management assessment, planning and direct services to consumers to enable access and benefit from required/indicated community services and resources in support of consumer recovery and community integration goals.
Administer Quality of Life Scale to members for use in identifying needs and personal goals.
Facilitate wellness themed education and support groups to members and in community settings.
Complete reports to be submitted to supervisor on a timely basis.
Meet with supervisor on a regular basis.
Be familiar with community resources and refer clients as indicated.
Coordinates his/her work with the BFSHC Manger, Wellness Coordinators and with consumer Facilitators, drivers, other employees, volunteers and other BFSHC consumers as needed.
Provides back-up for the BFSHC Manager as needed.
Treat all consumers with respect and dignity.
Attend all required meetings and trainings.
Follow agency policies and procedures.
Performs other duties as requested.
Qualifications:
A Bachelor's degree in a related field, such as:
Social Work (BSW)
Psychology
Public Health
Nursing
Human Services
1-2 years of experience in case management, mental health support, or community health work
Good communication/interpersonal skills;
Leadership abilities;
Ability to work collaboratively with others;
Good driving record and licensed to drive in N.J.
