  • Initiate and implement personalized physical therapy programs based on physician orders

  • Maintain detailed evaluations, progress notes, and discharge summaries

  • Collaborate with nursing and rehab staff to enhance residents' functional outcomes

  • Contribute to interdisciplinary care planning meetings

  • Maintain treatment documentation, track patient statistics, and request necessary supplies

  • Participate in or lead staff training and education

  • Provide leadership support in the absence of the Director of Physical Therapy

  • Graduate of an accredited Physical Therapy program

  • Strong interest in working with
