Job Summary:
The Providence Center Crisis Stabilization Unit (CSU) provides short-term care for patients experiencing an acute psychiatric and/or substance use crisis that could escalate to a point of requiring hospitalization. As an alternative to hospitalization or as a step-down for a transition between levels of care, the CSU’s services focus on stabilizing clients and addresses their mental health and substance use needs.
Duties and Responsibilities:
The Providence CenterCSU Evening Nursewill conduct follow up assessments and evaluate client health care data collected through interviewing, observation, record review and team input.
Analyze assessment data in conjunction with prescriber and team to formulate the basis of nursing care. Identify expected outcomes of care, individualized to the client, with the ultimate goal of improving client’s overall health status.
Administer medications per prescriber’s orders and monitor lab results as indicated.
Collaborate with the staff members in providing care to clients.
Maintain cooperative relationships with clients, staff and the public.
Complete daily activity logs.
Complete initial assessment forms.
Complete progress notes with appropriate format.
Develop and implement a clear, specific, behaviorally oriented treatment plan with ongoing consumer and team input and with consumer signature.
Complete insurance documentation as required.
Complete required reports and evaluations.
Follow required office procedures in a cooperative manner.
Initiate and process incident reports as per Center guidelines.
Adhere to Infection Control protocols.
Other duties as assigned.
Requirements:
Rhode Island/Compact License Registered Nurse preferred.
Associates Degree in Nursing required. Bachelor of Science in Nursing preferred.
Knowledge and experience with psychotropic medication, and ability to work well with professionals from other disciplines in a team approach.
Experience with addictions populations preferred.
Experience with managed care populations preferred.
