Job Summary:
Under the supervision of the Team/Program Manager, the home healthcare RN is accountable for effectively managing or caring for a caseload of patients from admission to transition into the community, involving interdisciplinary support and referrals when appropriate. This position coordinates the interdisciplinary care, monitors and adjusts treatment to achieve positive therapeutic outcomes, and identifies opportunities for process improvement. These responsibilities will be carried out in accordance with organization policies/procedure and standards consistent with Federal, State and accreditation regulations.
Specifications:
Equivalent education required for RI licensure required,
Bachelor’s Degree (min) in relevant discipline strongly preferred
