Job Summary:
The Providence Center helps adults, adolescents and children affected by psychiatric illnesses, emotional problems and addictions by providing treatment and supportive services within a community setting.
The Providence Center Clinical Therapist - Behavioral Outpatient - Fee For Service - with provide outpatient psychotherapy.
Duties and Responsibilities:
Perform a comprehensive psychosocial assessment of assigned consumers.
Complete crisis assessments, initial assessments and integrated summary forms.
Develop and implement a plan of care with ongoing consumer input.
Provide individual mental health and/or substance abuse treatment and crisis assessment.
Perform record keeping in accordance with Health Information Services and other Center requirements.
Other duties as assigned.
Requirements:
Master’s degree in Social Work, Human Services, or related field required.
Candidate must haveor
be in the process of obtaining Independent license (LICSW and/or LMHC).
Insured auto and valid driver's license.
