Key Responsibilities:

- Perform initial client evaluations within 48 hours of referral, ensuring thorough documentation and compliance with agency policies.
- Collaborate with the interdisciplinary team, communicating plans, progress, and changes effectively to physicians and case managers.
- Provide education and counseling to patients and their families about physical therapy needs and community resources.
- Engage in discharge planning and contribute to quality improvement initiatives, using tools like OASIS for assessment and outcome analysis.
- Ensure a safe environment for clients and coworkers, adhering to safety and infection control practices.
- Supervise and evaluate home care team members, including aides and therapy assistants, as per agency guidelines.
Qualifications:
- Graduate of an accredited Physical Therapy program recognized by relevant medical and educational authorities.
- Current state license/registration as a Physical Therapist.
- A minimum of two years’ experience in the healthcare field, with a preference for home care experience.
- Current CPR certification and a valid driver’s license with insured automobile.
- Strong communication skills and the ability to work independently.
