Community Pharmacist - Adams Health Center - Full-Time - Rotating

Full-Time: 36 Hours/Weekly
Rotating Shifts- Pharmacy hours are 7am to 6:30 pm
Holiday Rotation and On Call hours
General Summary
Provides pharmaceuticals and drug information as needed for adequate patient care according to professional standards and practices.
Essential Functions:
- Compounds pharmaceutical preparations and/or labels and packages medications and pharmaceuticals. Assists with and/or supervises IV preparation and/or delivery.
- Advises patients and medical/nursing staff on use of various medications including dosage, side effects, composition, drug-to-drug interactions, drug-to-allergy interactions and drug-to-food interactions.
- Ensures proper controls and records of narcotics and other controlled substances as required by state and federal law.
- Monitors drug therapy to identify, prevent or resolve actual or potential drug therapy problems. Addresses problems with physicians and/or nurses when indicated.
- Provides clinical pharmacy services as needed.
- Administers immunizations, reviews patient profiles for immunization qualification and makes that recommendation to the patient.
Common Expectations:
- Demonstrates knowledge of the principles of growth and development over the life span of the assigned patient population. Assesses and interprets patient age specific data and provides appropriate, age specific treatment. Provides direct patient care to assigned patient age group(s).
- Demonstrates a commitment to patient, visitor and staff by: complying with all applicable safety regulations; learning the impact of medical errors and methodology that will lead to reduction of errors; reporting actual and potential errors, as well as hazardous conditions; identifying opportunities to standardize processes and "error proof" systems that will lead to increased safety; and participating in safety education programs and root cause analyses as required.
- Maintains established policies and procedures, objectives, quality assessment, safety, environmental and infection control standards.
- Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.
- Provides outstanding service to all customers; fosters teamwork; and practices fiscal responsibility through improvement and innovation.
- Assists with and/or supervises personal care/long term care medication delivery.
- Assists in preparing required reports by compiling information.
QualificationsMinimum Education:
Bachelors Degree Pharmacy Bachelor's Degree RequiredWork Experience:
Less than 1 year Relevant experience. RequiredLicenses:
- Licensed Registered Pharmacist Upon Hire Required and
- Authorization to Administer Injectables for Pharmacist Upon Hire Required and
- Basic Life Support Upon Hire Required
