Case Manager - Waivered Services

Starting Salary Range: $51,517 - $55,036 + Office Benefit Package (Parking/Remote Expenses): $130/Month subject to change

RADIAS Health is hiring a full-time Waivered Services Case Manager to join the Waivered Services Case Management team!
The Waivered Services Case Manager assists clients on a waiver to access needed waiver and State plan services, as well as needed medical, social, educational and other services, regardless of the funding source. The Case Manager supports clients who have been deemed at risk of placement in a Nursing Facility, Neurobehavioral Hospital or ICF/DD, and sets up and coordinates the services and medical equipment necessary in order for them to live in the least restrictive setting possible. The Case Manager also provides ongoing monitoring of services, and serves as an advocate for the client when assistance is needed or services are not being performed as required. Goal development and service planning utilizes Person-Centered principles, and incorporates both what is important to and important for the client.
Hours: Monday - Friday 8 am - 4 pm or 9 am- 5 pm, 40 hours per week
Duties & Responsibilities
- Assist clients on the CADI, BI, EW and DD waivers in Hennepin County to access needed waiver and state plan services, as well as needed medical, social, education and other services, regardless of the funding source.
- Develop and implement a plan of care (CSSP) that is based on the MNChoices assessment results and within the county approved allocation for the client
- Inform clients and/or their legal guardians of service options, and assist clients in identifying potential providers and accessing services
- Plan and arrange for services and inform the county waiver contact of the proposed services to be authorized, including provider, service type, amount and frequency for entry into MMIS service agreement
- Provide services at the client's residence or other community locations, as specified in the CSSP.
- Provide crisis intervention services, as needed, including referrals to meet emergency needs for housing, food, clothing or financial assistance.
- Respond to all Vulnerable Adult reports in a timely manner of receipt of report. All responses will be documented and available for Hennepin County to review upon request.
- Determine client needs for residential treatment and select appropriate placement homes or facilities.
- Monitor client progress towards established service goals and determine termination/continuation of service.
- Consult with departmental staff, other county departments and community service professionals to develop and provide appropriate and timely services.
- Maintain accurate case records and complete forms for documentation of eligibility, treatment cost reimbursements, service referrals, evaluation of client outcomes and general case management purposes.
- Track and provide data used to monitor trends and developments relating to client and program needs, and to monitor agency outcomes.
- Track billable time worked, by client, for reimbursement
- Bachelor's degree in behavioral sciences or related field OR
- Bachelor's degree in any major AND one year of experience as a social worker, case manager or care coordinator
- Must have car, auto insurance and driver's license.
